We will start with a historical perspective on reimbursement and length of stay concerns, followed by a definition and discussion of what constitutes avoidable days. Methods for identifying and categorizing avoidable days will be reviewed with the goal of developing metrics (codes and categories) to identify opportunities for length-of-stay reduction and measuring the outcomes of interventions. We will then look at the concept of patient flow and reactive vs proactive approaches to improving the movement of patients through the system.
We will then look at the role of case management in identifying opportunities and achieving institutional objectives of timeliness, efficiency, and fully reimbursed care. and how case management aligns with these objectives and with patient flow initiatives.

Length of stay has always been one of the primary metrics for hospital case management. More recently, the Centers for Medicare and Medicaid Services (CMS) has added a new value-based measure called the “Efficiency Measure,” which rates hospitals on their cost and length of stay.
